The algorithm did exactly what it was designed to do. The problem was that nobody had documented what it should do differently when market conditions changed.

Amazon's AI recruiting tool was trained on 10 years of historical hiring data. That historical data reflected existing patterns in who the company had hired for technical roles — patterns that skewed heavily male. Because the hiring process hadn't been documented with explicit parameters around what "best candidate" should mean, the AI learned from what the process had produced. It encoded and accelerated the biases of the past rather than the intentions of the present. The tool was scrapped. The lesson: you cannot automate a process you don't fully understand, and if the process was already biased, automating it at scale makes the problem dramatically larger.

What AI-Ready Documentation Actually Requires

Documentation that works for humans is not the same as documentation that works for AI. Human readers can tolerate ambiguity, follow implicit context, and ask for clarification. AI systems cannot. They will use whatever they find, and they will use it confidently.

AI-ready documentation requires five things that most organizations' existing documentation doesn't have.

Defined scope boundaries. Each process document needs to be explicit about where the process starts and ends. AI systems "bleed" adjacent context together when scope boundaries aren't clear, generating outputs that mix elements of different processes in ways no human reviewer would have intended.

Verified accuracy. The people who know whether documentation is technically correct — engineers, subject matter experts, process owners — need to have explicitly signed off on it. AI cannot assess whether the steps it's following are correct. It will execute incorrect documentation at scale without hesitation.

Consistent structure. Documents need to follow predictable formats so AI systems can identify what constitutes a header, a step, a requirement, an exception, and a decision point. Documentation with idiosyncratic formatting produces unpredictable AI behavior because the system can't reliably identify what type of information it's reading.

Metadata that establishes currency. Versioning, revision dates, and explicit "source of truth" identifiers tell AI systems which version of a document is current and which is superseded. Without this, the AI treats a 2019 process document and a 2025 revision as equally valid and may draw from either depending on which it retrieves first.

Active retirement of obsolete content. Outdated documentation left in knowledge bases poisons retrieval results. An AI asked about a process will surface the outdated procedure alongside the current one and may weight them equally. The only solution is a disciplined process for retiring content when it's no longer accurate — which requires knowing, in the first place, that it exists.

The Five Levels — and the Level That Actually Matters

Process maturity models typically describe five levels of organizational capability. Most organizations fall into the first two levels without realizing it.

Level 1 is chaotic. Processes are ad hoc, unpredictable, and entirely person-dependent. Level 2 is repeatable in a loose sense — the same person tends to do the same thing the same way, but nothing is written down. Knowledge lives in individuals. Level 3 is defined: processes are formally documented as standard operating procedures and are consistent enough that a qualified person who has never done the task before can execute it successfully from the documentation alone. Level 4 adds quantitative management — metrics and controls that track performance against documented standards. Level 5 is optimizing — continuous improvement driven by feedback from the data Level 4 produces.

The gap that matters is between Level 2 and Level 3. Most organizations believe they're at Level 2 or 3. In practice, most are at Level 1 for a significant fraction of their critical processes.

The test for Level 3 is specific: can a qualified person with no prior exposure to this process execute it successfully using only what's written down? If the answer is no — if new hires still need to shadow experienced staff, if the documentation requires verbal explanation to be useful, if the document ends with "ask Sarah if you have questions" — the process isn't at Level 3. It's tribal knowledge with a PDF attached.

Level 3 is the minimum requirement for process improvement. You cannot apply Lean or Six Sigma to a process that isn't defined well enough to have a standard. You cannot measure variance without a baseline. You cannot improve what you don't understand at the level of documented steps. Where to Start

The most common failure mode in documentation initiatives is starting too broad. Organizations attempt to document everything simultaneously, make slow progress across dozens of processes, and abandon the effort after 60 days when the output feels insufficient relative to the investment.

The approach that works starts with the highest-stakes processes — specifically the ones that fit one or more of these criteria: they're currently causing visible operational problems, they're owned by one or two people whose departure would be disruptive, or they're targeted for automation in the next 12 months.

For each of those processes, the documentation effort has a clear sequence. Start by mapping the current state — what actually happens today, not what should happen. Interview the people who do the work, not just the managers who oversee it. These are different accounts of the same process and both are required. The people doing the work know the edge cases, the exceptions, and the informal workarounds that have accumulated over time. Those need to be captured.

Then define what the process should be — the Level 3 standard that a qualified person can execute from the document alone. Test it by having someone who wasn't involved in writing it try to follow it. Every point at which they need verbal guidance is a gap in the documentation.

Assign an owner for each documented process. That person is responsible for keeping the document current as the process evolves. Without ownership, documentation becomes stale within months and joins the pile of outdated content that creates the AI poisoning problem described above.

Then measure. Document the baseline metrics before changes are made — support ticket volume, time-to-completion, error rates, onboarding time. The organizations that are AI-ready today didn't become AI-ready by buying AI. That investment compounds. Every process documented is a process that can be measured, improved, and eventually automated.
